Submit your vaccination verification.
Students who are fully vaccinated and upload documentation via the exemption request form linked below will be automatically entered in the 70 in a Phlash campaign. This opens the door to win various prizes, including UW-Green Bay merchandise, events tickets, and even scholarships.

Vaccination Deadlines

The 70 in a Phlash campaign ends on Sunday, October 31, 2021.
 
Two-Dose Vaccine Recipients:

Students who receive a two-dose vaccine (Pfizer or Moderna) will need their first dose administered no later than Sunday, October 10 to receive their second dose by Sunday, October 31. They must also complete their Vaccination Verification form on the same day.

Single-Dose Vaccine Recipients:

Students who receive a single-dose vaccine (Johnson & Johnson) may receive their shot as late as Sunday, October 31, as long as they complete their Vaccination Verification form on the same day.

While we encourage all of our students, faculty and staff to get the Covid-19 vaccine, it is not mandatory.

It is not a HIPPA violation if a student shares their vaccination status voluntarily.

Credit will be applied to your SIS account for the Spring 2022 semester.

You will receive a refund on your tuition in the form of credit.

We understand there are a variety of reasons why people choose not to get the Covid-19 vaccine. However, in an effort to maintain a healthy and safe environment for the entirety of our university community, we are encouraging everyone who is willing to receive the vaccine to do so. We believe that incentivizing the process will increase student participation. No student will be penalized for remaining unvaccinated.
