Registration Forms

Certain courses have specific requirements that students must meet before they can enroll. When a student has not met the requirements placed on a course, the form(s) below may help! It’s important to complete these forms accurately to avoid registration errors. To expedite the form completion process, please make sure you have the following:

Complete your payment agreement in SIS. Learn more HERE.
Correct 7-digit Student ID Number
Complete Class Details
Instructor’s name and UWGB Email Address

Course Registration Override Form (For Class Prerequisite Waiver, Instructor Consent, Closed Class Waiver, Late Add Waiver and Time Conflict Waiver)
Audit/change grading basis (For non-degree-seeking audit students)
