Forms

Dynamic Forms (e-forms) require a network ID and password. If your network ID is no longer active, each e-form will give you the opportunity to create an ID to complete the form.

New, Returning, and Graduate Special Students

Apply to UW-Green Bay Graduate Programs: Please complete this form if you are a:
- UWGB undergraduate student, not entering an Accelerated Track, but interested in taking a graduate course.
- Prospective students interested in taking one class or completing a certificate program
- Degree-seeking students interested in applying to a masters or doctoral program.
Discontinued Student Re-admit Form: For UW-Green Bay graduate students admitted into a degree program, have not taken a class for one to three semesters, and are ready to return to their program.
- Students who have been away from the university for four or more semesters must reapply to their program.
Approval for Admission to an Accelerated Track (GR-A): For UWGB undergraduate students requesting enrollment in cross-listed courses for graduate credit in one of our Accelerated programs.

Graduate Course and Degree Forms

Completion Deadline Extension Form: To request extending the time limit to complete a graduate degree.
Course Registration Override/Late Add: For adding a course to an existing load of credits after the add deadline, or to request approval to register for a course as an admitted Graduate Special student.
Course Substitution: For approval to use one course in place of another toward your graduate program.
Credit Overload: For requesing additional credits beyond the maximum credit load.
Declaration of Masters Degree (GR-1): For students confirming a specific track or emphasis within their graduate degree program.
Graduate Assigned Study (595 and 696): For graduate-level instruction in a scheduled 300 or 400 undergraduate course if a corresponding graduate option does not exist.
Independent Study: For independent study registration.
Internship: For internship registration and to include internship titles on student transcripts.
Suspension Petition: For requesting a waiver of academic suspension status in order to be allowed to continue at the University.
Special Petition General Form: To request consideration that cannot be fulfilled from the above forms.

Dissertation, Project, and Thesis Forms

Approvals for Candidacy for a Graduate Degree Program (GR-2): For students completing a dissertation, project, or thesis. This form confirms that students have a faculty committee and have an approved research proposal.
Request for Dissertation Presentation (GR-3)

Approval of Dissertation Presentation (GR-4)
Approval of Project or Thesis Presentation (GR-4)
Exception to 1-credit Thesis Defense Requirement: For defending a thesis after the last day of classes of your graduation term.
GRAD 693 Course Approval Form: Zero credit course for students accessing university resources while completing a Project or Thesis.
GRAD 893 Course Approval Form: Zero credit course for students accessing university resources while completing a Dissertation.

Graduation and Program Completion

Application for Graduation: Students must apply for graduation one semester before they intend graduate.
Graduate Certificate Declare Form: For degree-seeking graduate students who would also like to transcribe their graduate certificate on their transcripts.
